The overall purpose of this job is to ensure to medication is optimally used and resolve any emerging medication related issues in line with established protocols and the Hospital’s patient care objectives.
ROLES AND RESPONSIBILITIES
- Verify prescriptions and treatment sheets for ICU patients for accuracy and suitability and to contacting prescribing doctor to resolve any problems ensuring the safe, economical and rational use of medicines and other pharmaceutical products;
- Report on therapeutic drug monitoring on medicines with low therapeutic index and comment on dose recommendations;
- Serve as an active member of the Medicines and Therapeutics Committee, infection Control and ICU users committee;
- Participate in formulation of Hospital policies on medicine use such as infection control, disinfection policy, chemotherapy policy and antibiotic policy;
- Assist in the designing of appropriate prescribing documents;
- Provision of drug information to consultants and doctors;
- Support clinicians and nurses on pharmaceutical information;
- Partnership with doctors on individualised patient management;
- Patient evaluation in specialised cases such as renal;
- Update the pharmacy system on stock and drug details as appropriate;
- Guide patients on the dosage and use of medication;
- Run statistics for the pharmacy department;
- Participation in various committees in the contribution to the pharmaceutical body of knowledge;
- Interpretation of medication that requires lab assessments;
- Drug vetting and registration for approval for dispensation by the hospital;
- Monitoring the extent and root cause of drug errors and development of remedial actions on the work flow; and
- Any other responsibilities that may be assigned to the job holder by the supervisor from time to time
EDUCATION AND EXPERIENCE
- Masters in Clinical Pharmacy from a recognized institution.
- Bachelor’s degree in Pharmacy from a recognized institution.
- Must be registered with the Pharmacy and Poisons Board of Kenya and hold a valid practicing licence.
- Minimum of 3 years’ experience as a Clinical Pharmacist.
